67-year-old female patient
Revision, Treatment with MP Reconstruction System
Dr. Rafael J. Sierra
Department Orthopedic Surgery
Mayo Clinic, Rochester, Minnesota, USA
Case Description
Preoperative X-ray
- 1999 Primary left THA: Coxarthrosis
- 2007: Left THA revision for loose femoral component
- 03/2013: Sustains fall, unable to fully weightbear on left side
- X-rays show periprosthetic fracture, with appearance of loose femoral component and significant stress shielding of proximal femoral bone.
Preoperative X-ray
Course of Treatment
Postoperative X-ray, 3-month follow-up
- 05/2013: Revision femoral component
- Type 4 femoral defect Paprosky Classification
- Wagner style osteotomy to facilitate femur exposure, implant removal and implantation of Link femoral component
- Reinforcement of distal bone with lateral strut graft.
- Components of the MP Reconstruction System:
- Stem: Cementless, length 330 mm, prox.diameter 27,5 mm
- Neck Segment: XXL, length 65 mm, CCD 126°, offset 40 mm
- Spacer: Length 10 mm + 20 mm = 30 mm
Conclusion
Postoperative X-ray, 6-year follow-up
- 6 year follow-up Radiographs show implant fixation, reconstitution of proximal bone
- The modular MP Reconstruction System allowed a management of a complex femoral deformity with periprosthetic fracture and reconstitution of poor proximal bone
- The result was excellent allowing the patient fully weightbearing and return to activities as tolerated
- She passed away 7 years after THA revision with a fully functional extremity
Postoperative X-ray, 6-year follow-up
